Physical Therapy
Strength, balance, gait, and joint range of motion — built up session by session. Our PTs are experienced in post-surgical recovery, fall prevention, and chronic condition management.
From a few weeks of focused recovery to a permanent place to call home — our programs flex with the people we serve.
Licensed nursing care delivered by a team that knows each resident by name and by history. Medications, wound care, vitals, and clinical judgment calls happen with people who already understand the person in front of them.
Each resident has a charge nurse and a primary care physician (yours, or one of ours) who is kept in the loop with regular updates. We coordinate directly with hospitals, specialists, and home health to keep transitions smooth.
Most short-term residents come to us straight from the hospital after surgery, a fall, a stroke, or another event that calls for a structured recovery. The goal from day one is clear: get you back home, safely, on your own terms.
Our rehab gym is large, bright, and stocked with equipment ranging from simple parallel bars to advanced gait trainers. Therapy sessions run six days a week, and your progress is reviewed weekly with you and your family.
For residents whose daily needs are too much for home but who still want a real life around them, Colonial Manor offers a long-term home in every sense of the word. Pictures on the wall, a favorite chair from home, a routine that fits the person — these are not extras here, they're the baseline.
Our long-term care residents share meals, plan outings, attend services, and build friendships. Our staff turnover is low, which means the aides and nurses who know you today will likely still know you next year.
Three disciplines, one team, and a shared belief that small daily wins are what real recovery looks like.

The activities that make up daily life — dressing, cooking, climbing into a car, getting back to a hobby. Our OTs rebuild the skills that matter to you specifically.
Communication and safe swallowing after stroke, neurological events, or extended intubation. Patient, persistent, and measurable progress.
For residents living with Alzheimer's, dementia, or other cognitive changes, our memory support approach focuses on calm routines, familiar faces, and environments designed to reduce confusion rather than increase it.
Staff in this area are specifically trained in dementia-informed care — meeting residents where they are emotionally, redirecting gently, and finding moments of joy through music, touch, and gentle movement.
Family caregivers carry an enormous load. Respite care is a short stay — a few days to a few weeks — that gives your loved one a comfortable, supervised place to be while you travel, recover from your own surgery, or simply rest.
Respite residents enjoy the same meals, activities, and clinical support as our long-term residents. Many families use respite stays first as a low-pressure way to get to know us.
When the focus shifts from curing to comforting, families deserve a setting where that transition is honored with the same gentleness as any other phase of care. We work alongside the hospice provider of your choosing — most major Tulsa hospice agencies are familiar partners.
Our role is to keep the room peaceful, the resident comfortable, and the family supported. Visiting hours don't exist during these times: family is welcome whenever, for however long.
Programs and capabilities that go beyond standard skilled nursing — coordinated by our medical director and on-site clinical leaders.

Oxygen therapy, breathing support, and respiratory recovery for COPD and post-pneumonia residents.
Daily monitoring, insulin administration, and dietary planning to keep blood sugar consistently in range.
Coordinated medication review and non-pharmaceutical strategies to keep pain at a livable level.
We work with Medicare, Medicaid, most major commercial insurers, long-term care insurance, and private pay. Our admissions team will verify your specific coverage and walk through any out-of-pocket implications before admission.
For short-term rehab admissions directly from a hospital, we often complete admission within 24 hours of the referral. Long-term care admissions follow a brief assessment process — usually within a few business days.
Yes. We coordinate with your physician of record. We also have a medical director and visiting providers available for residents who prefer on-site care.
Mornings begin with breakfast and personal care, followed by therapy sessions or activities tailored to interest. Afternoons typically include rest, social time, and a second activity block. Evenings are quieter — dinner, visiting, and an early bedtime for those who want it.
Visiting hours run 9 AM to 8 PM daily, and we accommodate special-circumstance visits outside those windows. Family is welcome to join meals or activities at any time.
Absolutely. Rooms are intended to be made personal — favorite chairs, photographs, quilts, and small furniture are all welcome. We'll work with you on what fits comfortably and safely.
Our admissions team will walk through the options with you, no pressure and no obligation. A short conversation often makes the path forward clear.
